New US Passports (16 Years and older)
To get a new US passport quickly, applicant must meet the following qualifications:

-- Applicant is a US citizen over 16 years old AND;
-- Applicant has never had a U.S. Passport issued before OR;
-- Applicant was under 16 years old when most recent passport was issued OR;
-- Applicant has not had a U.S. Passport issued for over 5 years.
--Applicant is not  behind in child support payments, he/she may not be able to get a passport.

Applicant must gather documents listed below to take to an Acceptance Agent.

-- An Acceptance Agent is a government employee who will verify the identity of the applicant.
-- Acceptance agents are at Post Offices and Court Houses. Click here to find an Acceptance Agent near you.
-- Applicant must indicate that he/she is using a "hand carry" service. The
--Acceptance Agent will have the applicant sign the application; seal documents in an envelope and return the sealed envelope.
Sealed envelope is forwarded to Wevisa for processing.

Note: Passport applicants between the ages of 16 and 17 must appear with one parent to co-sign the application.For applicants under 16.

Certified birth certificate issued by the city, county or state*

*A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office, which must be within 1 year of your birth. Please note, some short (abstract) versions of birth certificates may not be acceptable for passport purposes.

Passport Photos
  

Two (2) identical passport photographs. Go to a passport photographer to have the photos taken. Homemade photos are not acceptable and most often causes a delay in passport issuance.

Proof of Departure
  

Submit a copy of your computer-generated itinerary or your ticket from an airline, travel agency, or online booking agency showing your departure from the U.S. FAX or photocopies are acceptable. Please include one copy inside the sealed envelope and one copy outside of the sealed envelope.

Government Fees
  

Two checks or money orders, one payable to U.S. Department of State for $170 and the second for $25 payable to the courthouse or post office (the acceptance agent fee).

Note: Please include your date of birth on the memo line of the check. Note: Do not use starter checks for new bank accounts.

Authorization Letter (PRINT TWO)
  

A letter of Authorization giving us the authority to submit your application and pick-up your passport when issued. Provide two originally-signed authorization letters - One copy should be inside the sealed envelope and one copy outside the sealed envelope.

Note: Each copy MUST contain your original signature preferably in blue Ink. Fax or photo copies of your signature are unacceptable - both letters must be originally signed.
